Waiver and Release of Liability

By registering for and/or participating in this event*, I, the undersigned parent/guardian, acknowledge and agree to the following:

Assumption of Risk

I understand that participation in this event involves certain inherent risks, including but not limited to personal injury, illness, property damage, or other loss. I voluntarily assume all such risks, whether foreseeable or not, that may arise from participation in the event.

Parental Responsibility

I acknowledge that I am solely responsible for the supervision, care, and conduct of my child(ren) during the event. I agree that I will remain with and supervise my child(ren) at all times.

Release and Hold Harmless

I, on behalf of myself and my child(ren), hereby release, waive, discharge, and hold harmless Lorain with Littles, Lorain Historical Society, and ideastream public media, as well as their directors, officers, employees, volunteers, partners, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to participation in this event, whether caused by negligence or otherwise.

Photo/Media Release

I grant permission for Lorain with Littles, Lorain Historical Society, and ideastream public media to use photographs, video recordings, or other media of myself and/or my child(ren) taken during the event for promotional, educational, or informational purposes, without compensation. I may revoke this consent in writing prior to the event if I choose. (Email us at lorainwithlittles@gmail.com)
